Your Digital Companion Before and After Orthopaedic Surgery
To help you feel informed, supported, and confident throughout your orthopaedic surgery journey, we provide access to the Patient Optimised Pathway (POP) app , a simple, easy-to-use digital companion that guides you through every stage of your care.
Once your surgery is confirmed, our team will register you for the app and provide a unique access code to log in and get started. Downlod the app here:
Why are the questionnaires important?
Before surgery, you’ll be asked to complete a brief survey in the app. This gives your clinical team important insights into your health, mobility, and personal goals — allowing for more personalised care.
Follow-up surveys are then sent at:
- 6 weeks after surgery
- 6 months after surgery
- 12 months after surgery
These check-ins help track your progress, highlight any concerns early, and guide your care between appointments.
